The inquiry form collects only the information needed to respond.
The form requests basic contact information and a short description of the student’s current course, goals, and requested support. Sensitive student documents should not be submitted through this form.
Information Submitted
The inquiry form may collect the contact person’s name, relationship to the student, email address, and phone number if voluntarily provided; the student’s grade and current math course; the service being considered; the student’s school, if voluntarily provided; how the person submitting the form heard about Kennedy Nguyen Mathematics; and a description of the student’s current goals and priorities.
How the Information Is Used
The information is used to review and respond to the inquiry, determine whether current services and availability match the request, answer follow-up questions, and arrange a fit conversation or other next steps.
Information submitted through the inquiry form is not sold or used for unrelated marketing.
Form Delivery
Inquiry submissions are sent to kennedynguyenmath@gmail.com through FormSubmit, a third-party form delivery service. FormSubmit states that submissions may be retained for up to 30 days for delivery and recovery.
Student Documents and Sensitive Information
Do not upload or email full academic records, identification documents, medical information, disability documentation, or other sensitive student materials as part of the initial inquiry.
If coursework or other student materials are needed later, separate instructions will be provided after the initial inquiry has been reviewed.
